sounds like the usb stick is corrupt - take it back to your administrator and get them to reinitialize it.
Perhaps it was removed while it was active at some point, without using the "eject" option in Windows?

I downloaded "McAfee Encrypted USB Manager 3.9" but I never use that before. I don't know what Database & LDAP Setting was. I am running a small company and I am the IT guy with some basic knowledge on data security. This Encrypted USB is my first try on the "data protection" device.
How can I turn this USB to "normal" USB without key in any password?
How can I restore it to factory default? (remove all data already stored inside is all right with me)
It's not possible to turn it into a "normal" insecure stick - it's a really expensive, hardware encrypted device, not designed to be used any other way.
You might be able to reset it - it depends on how you initialized it, and whether you still remember the recycle code or not. Probably best to contact the company you bought it from and get some training or professional support?

I remember the code I put in during the first time I received the USB. However, there is no program for me to access the USB.
I would get MXIRECYCLE.EXE this will reset the key.
What software did you set it up with? EPO, or the stand alone USB Manager?

I Google it can can't get any site provide mxirecycle.exe.
Neither EPO, nor the stand alone USB Manager. It was the original software come with the Encrypted USB device. The very first time plug in to PC and it run the software automaticlly. It prompt to enter the password for the 1st time. I key in the password for the 1st time, record it in a piece of paper, that's it.
McAfee encrypted keys are made by MXI http://www.mxisecurity.com/en/ , I had a similar issue before and I openned a ticket with them and they sent me the file. I don't see a way to upload a file here, if you can provide me with your email address, I will be glad to send it to you.
