Please take a look at the orion.log and perhaps the epoapserver.log on the ePO server at the time of the error.
This may give some indications of the actual errors involved.
I found the reason.
The server had no more free diskspace.
The cause is the following directory
c:\Program Files\McAfee\ePolicy Orchestrator\DB\RepoCache\Current\VSCANDAT1000\DAT\0000\
This contains all the old versions of dat files (+8GB).
Shouldn't they be cleaned up automatically?

Yes they should be deleted.
That sounds like issue #14 resolved in ePO 4.5 Patch 1.
Issue: The Agent Handler Repository cache grew continuously. (Reference: 512678)
Resolution: The Repository cache now purges obsolete content.
Covered in more detail in Knowledge Base article KB67585.
I'll try the patch during our next maintenance.
