I am trying to install Epo 4.5 on windows server 2003 R2 server. and I got the following errors from core-install.log

Best experience is performing upgrades using admin account which is server admin, SQL DB admin and ePO global admin, all at the same time. Such account can be used for the upgrade only then revert back to which ever account you want for operations. I find that this approach helps a lot with permissions/connection issues.
