ePO Policies not enforcing

Question time again.  After routine checking of some of our endpoints, it appears that a number of custom-set policies are no-longer enforcing.

Most (I've not checked 100%) of the default policies are in place and being pushed/enforced at the endpoint level, but the custom on-access policies I've created dont seem to be.

It appears at the moment that the endpoints are getting the "Settings for: Server" version of a policy, rather than the Workstation version, given which preferences are saved.

Endpoints are communicating with ePO server OK and do not appear to have issues with reporting in or responding to Agent wakeups.

Policy enforcement is set on ePO server without any problems and is set to replicate/inherit correctly in the system tree.

Has anyone encountered this or have any ideas what might be the issue?
