I don't know what you managed to do, but here are the rule i currently use. Delete the old rules and all the user-defined and settings so they don't conflict...or rename the new settings on import.

How should we modify our site review to include all those user-defined variables (Title, category, uploader, keyword...?

There are many different methods that people have implemented for site review. I cannot say specifically how you are doing it.

basically, find the site review components in your implementation and add the user-defined fields in the same way, along with your other fields.

On mine, i have some form fields that are base64 encoded on the original block page and are submitted and processed on the subsequent forms and rules.

You could just add the extra fields, or create a seperate YouTube site review mechanism on just the block pages presented for a YouTube block.
