Need information why an instance that mcafee failed to deleted or respond properly on threat.

Notice on Threat Handle is FALSE.

Should I worry?

what are the mitigation?

You need to give more details as to the actual threat - the MD5 etc

file name is immaterial - did you run it through virustotal or getsusp?

It's not uncommon for antivirus products to miss new malware samples that vendors haven't had a chance to analyze. That's why products like Threat intelligence exchange and ATD exist... You can submit the file to McAfee for analysis:

I agree that why we have different layer of protection....but the malware details was discovered way back 2013..

Can you share more info from that event? If it's generating an even then it's probably picking up the file, but it's not able to remove/clean it for whatever reason.

that is the only event that failed to delete the threat.. others are successful (with different threat name and infected file).
