I have a couple of questions for you on this one:
1. What browser are you using?
2. If IE8+, are you running in Compatibility Mode when accessing the MQM?
Some browsers, most notably IE8 and above like to not display content in forms properly. It is necessary in these cases to run in compatibility mode. If you're using something else (Firefox/Chrome), then I would recommend calling in to Support and getting a ticket opened to investigate this one.
check if LDAP server is populated under MQM "Setting and Diagonstic->Communication->LDAPServer.
Apply Rollup1 Patch as well .
We are in compatibility mode and doesn't help. Does not work in Mozilla or Chrome either. Have had one ticket opened and no help so far.
LDAP is populated as we are sending email digest from MQM every evening.
Patched to current already.
